Submission URL issue

I'm trying to submit a survey back to a hosted feature service as described here. However I am receiving the below error. I believe it may be because my service contains both OBJECTID and GLOBALID - how do I represent these in my XLSForm?

Hi Andrew,

The objected and globalid fields are accounted for automatically by Survey123. Are the field names as expressed by the feature service identical to the naming of the fields (questions) in the form? What is the configuration of your Server?

Yes - my hosted Feature service has the following fields:

and my Survey XLSForm is constructed as shown below:

The only variations are the two images in my XLSForm which are held as attachments in my Feature Service.
