      <description>&lt;HTML&gt;&lt;HEAD&gt;&lt;/HEAD&gt;&lt;BODY&gt;&lt;P&gt;Hi Everyone,&lt;/P&gt;&lt;P&gt;&lt;/P&gt;&lt;P&gt;I am building a Chain of Custody form in Survey123 and am looking for any examples, ideas, etc. the community may be able to provide.&amp;nbsp; My initial thought is to have 2 forms.&amp;nbsp; The first would be used to describe the object in transit.&amp;nbsp; The second would be the Chain of Custody form that would be filled out every time the object changes hands.&amp;nbsp; Thanks for your help!&lt;/P&gt;&lt;P&gt;&lt;/P&gt;&lt;P&gt;Derrick&lt;/P&gt;&lt;/BODY&gt;&lt;/HTML&gt;</description>
