Survey will not create

I am trying to create a new survey using the web designer but it will not create. It gives a very long error message.

I have been using survey123 for some time now and had no issues.

Hi Will,

We just released a new update of the Survey123 website. Could you refresh your browser cache (pressing the Ctrl key and the F5 key together normally does this)?

That didn't work. I'm wondering if it may be related to my arcgis online user profile.
