ArcGIS Server Query Issue

Hi,

We are having a strange issue with ArcGIS Server doing query requests. Requests like the following work 50% of the time.

http://geoappext.nrcan.gc.ca/arcgis/rest/services/MMS/MW_E/MapServer/0/query?f=json&where=1%3D1&returnGeometry=true&spatialRel=esriSpatialRelIntersects&outFields=*&outSR=3978&callback=dojo.io.script.jsonp_dojoIoScript_Smelters_and_refineries00._jsonpCallback

It alternates between success and failure. Why does it do that and what can we do to fix this? On failure, we get the message:

dojo.io.script.jsonp_dojoIoScript_Smelters_and_refineries00._jsonpCallback({"error":{"code":400,"message":"Unable to complete operation.","details":[]}});

Version: ArcGIS Server 10.1

Server setup: 1 server running Windows Server 2008 R2 with IIS and ArcGIS Server Web Adapter controlling 3 Windows Server 2008 R2 sharing the load for the services.
