Quick Capture failed to send record

I am in the process of re-vamping one of our QuickCapture applications. I've created the new application from scratch and am in the testing phase now. When I click on a button from my mobile phone I'm already receiving a Failed to send record message. Why is this happening?

When I click the Send via Email button I see an error: "Code":1000,"description":"Conversion failed when converting from a character string to uniqueidentifier." But the description field shouldn't get populated when a point is captured.

We had a similar problem where a QuickCapture form was working perfectly using an ArcGIS Pro published feature service.

I opened a ticket where the consultant resolved "client was using a traditional versioned dataset with quick capture which is not supported. https://doc.arcgis.com/en/quickcapture/help/guidedtour-prepare.htm"

However, this did not explain why it was working...then not, and would not send the capture data. There were a few updates to the App in this time.

The only way to resolve it was to create a Hosted Feature Layer from Portal and reconfigure.

Thanks for your suggestion! I just realized that I'm only having issues collecting info for one of my buttons which I have connected to a Workforce project. I connected the button to a view layer and that seems to have fixed my problem.
