We have ArcMap 10.4.1

I have a random, reoccurring issue where layers will dissappear from a map document after saving and closing. It's not a broken data link issue. The layers simply vanish from the map. It seems to happen in conjunction with an issue with the map template (toolbars aren't open, etc.). I've tried mxd doctor, but it doesn't help. Clues?
