I'm having trouble finding the correct resource I need.

I have a CSV file with addresses, lat and long, and values. I don't know how to format the CSV correctly to map the exact points and capture the values. Or maybe I have to do that separately? I don't know. Please help.

https://doc.arcgis.com/en/arcgis-online/reference/csv-gpx.htm

You need to have a clean table with column headings. ArcGIS Online will try to guess the fields and type, but you can correct it when you upload it.

If you think your table is not clean or is not working, see this article for generic issues that may be occurring:

https://support.esri.com/en-us/knowledge-base/problem-generic-errors-from-uploading-and-published-csv-000024821
