Crowdsource Reporter Submit a Report Error

Hi all,

I am receiving an error with the crowdsource reporter tool when clicking on the 'Submit a Report' button. The app hangs and throws this error in the console:

(replaced my organization portal with Xs)

This is on AGOL version 2023.2

It was working recently, and today was the first day I noticed the error. I tried reverting recent changes I had made (removed layers from map, removed filters, etc.) but have had no luck.

The reporting layer is a point layer with two fields to fill out with attachments enabled. There are two hidden fields for voting and moderation.

Any help would be appreciated.
