Crowdsource Reporter Form will not hide fields despite configuring pop-ups

Hello,

I have configured a crowdsource reporter app and created a web map to go with it. I have configured the pop-ups in the web map, however when I try to submit a comment in the reporter app it shows all the fields. I need some of the fields to not be visible to the public. I have tried configuring the pop-up in map viewer & map viewer classic, I have tried configuring a form in map viewer, and I have even tried republishing the hosted layer with ArcMap rather than PRO. What am I doing wrong??

I remember having a similar issue before, and after searching online, I found the information on configuring pop-up settings for feature layers in ArcGIS Online helpful; refer to the following documentation from Esri:
