GeoForm Add New Fields

I am trying to update a GeoForm I created. I added a few new fields to the base feature service however they are not showing up in the form editor. I remember needing to add the fields in the JSON for the application however I cant remember how to get to that. Can anyone help me.

Where is this geoform hosted? on AGOL or your own server?

we are required to have an inventory of all the curbs and ramps in the county and if they meet requirements. SO I made an application that any of the road crew could use a phone to enter the measurements or new points as needed. The web map also will let them edit the points already placed.

At the time I could not get the domain dropdowns to work and I needed it to be able to add photos. After fighting with it for a day I found this and it worked much easier. BUt editing it has been more tricky
