can we create data collection pop-up form?

I am going to make web application of field data collection for tree. i don't know how to make data collection pop-up form and how to be apply that it take in attributes table and save it then we can download in csv format. i am beginner in this. so can anyone help me for this kind of problems?

Hi,

If you are looking to collect data from the public or a specific set of users, you should first start with creating a hosted feature service. This data type supports multi-user editing when editing is enabled and shared. Also, take a look at the geoform web app. This template provides form based data collection, where entries are added to the feature service. you can also export the data to a csv from a feature service. check out these help links for guidance:
