Reporter app - drop down list as domain values

I am working on a simple reporting instant app, for the public to drop a point and leave a comment. I have an editable layer, with a category type field with domain values for the 'drop down list' and a field for comments.

I have built a form. In the app however, when a user goes to "report" a list comes up to choose "category 1, category 2, ... category 5" after picking one, THEN the report form comes up with the drop down list and comment line, name and email entries.

How do I skip the first request/filter on the report button? It would be ok if that "category 1, category 2" list was a list of domain values and then they would submit a comment (I can go modify the form).

Should I be using a related table and using the comment feature instead? The user still needs to drop a point.
