A simple app that only allows users to add points

It would be really helpful to have an app that only allowed a user to add a point to a map. No updating, no deleting, no editing/seeing attribute fields. Ideally, should be able to limit number of points to be added, and not see other's points.

I've tried embedding a variety of AGOL based apps in a REDCap survey instrument (REDCap is a widely used open source web based data collection/management for health data). I keep running into issues of attribute fields popping up, and/or unwanted editing tools, and/or only allowing one point to be added at a time.

I'd think this would be pretty simple, and maybe it is, but I haven't found a way to make it work. If there isn't a basic app for this, there should be.
