Why does smart form display field name twice?

I'm using a client's Enterprise instance to develop a form to perform inspections in the field using the Field Maps mobile app. When the forms open on the mobile app, there is the Display Name shown above the text entry field, but then there is some redundant text shown below field that is exactly the same as the Display Name. How do I eliminate this redundant text, I can't find it anywhere? See attached.

I figured it out.

That's the Description field. Someone had populated it with the same text as the Display Name. Removing the Description from the Feature layer doesn't remove it from the form though. The field has to be removed and reinserted from the form to remove that text.

You're correct. What version of Enterprise are you using? The description property wasn't added in the UI until after 10.9 was released.
