ArcGIS Experience Builder- Issue with Embed Widget and S123 Form

Hello Experience Builder Team,

We're seeing issues with the Embed Widget and our S123 form, please see below. We must use the embed widget (vs S123 widget) because of parameters that are being passed but are not in the data.

As you can see the Embed widget is connected to the data and the survey is rendering but with errors:

We're currently using this workflow in ArcGIS Dashboards but Experience Builder would be ideal because of it's tabular interface and robust filter widgets & search functionality. (Dashboards example below)

Unfortunately we need a resolution (or alternative workflow for non tabular editing in EB) quickly as this project is high profile and going live in the next week or so.
