I already created the form using connect (v3.12) and published it to my portal contents. I'm interested in the connecting feature and form. Our clients want to click a point in the map and generate a point with the information from the existing point. So the function seems the right way to go, but I can't add my Survey123 form to the app. I saw there is a bug showing the forms in the list in the portal 10.8.1. Our portal is 10.8.1 so I think it's a bug. Since we just upgraded the portal to 10.8.1, we are not planning to upgrade it again soon. I saw someone was able to use the survey widget with portal 10.8.1.
