map service view is blank

Map service preview is blank in ArcGIS JavaScript API from an ArcGIS Server map service but displayed successfully in Arc Map.

I'm currently working with ArcGIS Server 10.2 installed on a Windows Server 2012 R2. I had no problem installing the service and creating the site, i also can access to the Manager (via localhost).

Problem is, when i start the map service SampleWorldCities located in the root folder and try to visualize it by pressing the button on the thumbnail i catch a blank page like attached. any solution?

Can you confirm if there is any scaling set on the map service?

I am facing similar problem, in ArcGIS Server 10.2. I have successfully published a Feature Service, when I access this service from ArcGIS Desktop I am successfully able to see it. however when I view it using ArcGIS Server Manager by clicking on Thumbnail view of feature service it opens blank JavaScript API template in Google chrome browser.

And you are confirming that the layers are not scaled or turned off from the mxd level?

Yes, layers are not scaled or turned off in mxd.
