Data Reviewer toolbar not found

I have the ArcGIS Data Reviewer extension with my ArcInfo license..but when launching ArcMap and searching in the extensions or the toolbars i cant find it !

Any ideas about this weird problem ?

Please confirm if you have completed the installation of Data Reviewer. Data Reviewer has a separate install on top of ArcGIS Desktop and you will be able to see the Data Reviewer Extension in the Extension dialog only if the Data Reviewer is installed.

Also please confirm the version of ArcGIS Desktop and Data Reviewer you are using?

I didnt know that the Data reviewer had a separate installation, i have the ArcGIS desktop service pack 1, i found the installation of the Data reviewer service pack 1 but before installing it, i need to have the installaion of the Data reviewer 10.

Do you know where can i find the installation of the Data Reviewer 10 ?

I am using ArcMap 10 service pack 3. I have installed Data reviewer 10 but the toolbar does not show up in the list. Also, Data Reviewer does not show up in my list of extensions. I can start the Data Reviewer service and create a Data reviewer session in ArcTool box. Any ideas?

I am using ArcMap 10.0 (build 3600) ArcGIS Desktop Service Pack 3 (build 3600) I will check into the MySQL to Access installation
