ESRI Application Protoype Lab Demo Code Available?

I am logging on as a guest to view the demo apps and found one that would be perfect for me to use, however it does not have the code sample available to download like some of the others there. I was hoping that someone, :rolleyes:or the person who created this very useful app would see this post and direct me to the demo code for it or share it with me. I'm new:confused: to javascript so having a template to start with would be extremely helpful. This is the app I would like to modify to use:
