Previously we hosted this data on our own server so I was able to query the data from the Oracle database to populate the table but since we transitioned the data over to a hosted feature layer, I can't seem to find any examples or information on whether or not this is possible or if there are any other methods that allow for using a pdf document as a template.</P><P>The attached images shows a portion of the form.
