The error actually doesn't tell much other than the extent at which the caching fails. 1. Upgrade to 10.1 SP1 Quality Improvement Patch. Many bugs and other issues related to caching have been addressed with this patch. http://support.esri.com/en/downloads/patches-servicepacks/view/productid/160/metaid/2064 2. Examine status.gdb created in the cache directory. You can create a copy of status.gdb on your local machine and manually examine 'task status' and 'job status' feature classes by pulling them in ArcMap. These feature classes may convey valuable information (possibly an error) about the extent and level the cache creation is failing. You can then check for the elements in that extent which might be stalling the caching process. 3. The error may also result because of resource limitation on the server. You can experiment by increasing the no. of instances allocated to the Caching tools service. As a general rule the max instances can be n+1 where n is the no. of cores on the server machine.

Hello Mike, In case you want to stick to 10.1, I would suggest you to upgrade to 10.1 SP1 Quality Improvement Patch. Many bugs and other issues related to caching have been addressed with this patch. http://support.esri.com/en/downloads/patches-servicepacks/view/productid/160/metaid/2064 Also, a good approach in these cases is to examine status.gdb created in the cache directory. Stalled processes can perhaps be killed by manually removing the processes from the arcgisjobs > caching service directory. You need to stop the caching service before manually deleting theses jobs. -Best from Divyam.

Hello Tom, ArcGIS.com and Portal for ArcGIS.com map viewer do not support custom print templates. No matter what template you choose, you will always get the print in MAP_ONLY. A good approach would be to create a web mapping application like Basic Viewer template to share your content (webmaps) which support custom print templates. -Best from Divyam.

Hello Leah, Are you using ArcGIS Server Feature Service in ArcGIS.com webmap or is it an ArcGIS.com hosted feature service? A good test would be to consume your service in any other client you are familiar with.. For example a Flexviewer application. If its an ArcGIS Server FS then it might be related to some sort of a permission issue over the Database. In that scenario you might want to try and publish another feature class as a FS service on ArcGIS Server and then try and edit it through ArcGIS.com.

Is ArcGIS.com subscription account running on https? If not then you can try enabling ssl on arcgis.com as well. My organization>edit settings>security.. Have seen some issues with adding cross content, due to mis-match between the 2 protocols.
